I’m somewhat doubtful regarding how well the PA-5000 works (along with comparable units from Centurian). If the glans gets stimulated, it seems plausible that ejaculation and an orgasm could still happen.

So, for those using a PA-5000 or a Centurian tube—have you been able to cum while wearing one?
