Every KH is different with what they want and how they want to control you. Question 1. I am allowed out every 7 to 10 days for an orgasm. How I get that orgasm is up to her. Question 2. I have only been released once for chafing in 13 months of 24 / 7 . Got a stern talking to about it. She was mad because I was not taking care of her toy. Every day now after my shower I put on couple of drops of baby oil around the base ring and into the cage to keep it oiled.
When I am locked I remain continuously locked with no release until she wants me unlocked. Then I may be allowed an orgasm and locked again shortly after, or I might get a day or so free before being locked again, or she could decide to let me stay unchained for a longer period. The latter is usually the case when I have a business trip or some other reason making her uncomfortable with keeping me in.

If I'm chafed, the device comes off until things are healed. That's why it's important to test a device for several weeks before going long term.
